Home Health Physical Therapist

Methodist Hill Cntry Home Care is seeking a PRN Physical Therapist to provide compassionate home health services to patients in the community. In this role, you will perform comprehensive assessments, develop and implement individualized treatment plans, and collaborate with an interdisciplinary team to improve mobility, safety, and quality of life. You’ll educate patients and caregivers, document care in compliance with regulatory standards, and support holistic, patient-centered care in alignment with our mission and values. Flexible PRN scheduling; home health experience preferred and current state PT license required.
