La Villa Grande Care Center in Grand Junction, CO seeks a compassionate Physical Therapist to provide skilled rehabilitation services for senior residents in a long-term care and post-acute setting. Evaluate, plan, and deliver individualized treatment programs to improve mobility, strength, balance, and functional independence. Collaborate closely with an interdisciplinary team, educate residents and families, and document progress accurately.
