Full job description
Horizons Care Center is seeking a compassionate RN to provide skilled nursing care to residents in our long‑term care and short‑term rehab units. The RN will complete assessments, administer medications and treatments, monitor changes in condition, and coordinate with physicians, therapists, and families to support individualized care plans. Responsibilities include supervising CNAs, maintaining accurate documentation, promoting safety and infection control, and participating in quality improvement. The ideal candidate is patient‑focused, a strong communicator, and thrives in a collaborative, interdisciplinary team environment.
