Physical Therapist - Travel Contract

Minimum Requirements

• Bachelor's, Master's or Doctoral Degree in Physical Therapy from accredited schools approved by the APTA
• Active State License is Required to Start the Assignment
• BLS Certification May Be Required from AHA or ARC
• Active Maryland Physical Therapist license required to start.
• CPR (Basic) required to start.
• Hepatitis B required to start.
• TB required to start.
• Flu required to start.
• Physical Therapist checklist required.
• Physical Therapist competency exam required.
• Criminal Background required to start.
• OIG Search required to start.
• HIPAA required to start.
• Statement of Good Health required to start.
• Annual Review required to start.
• OSHA Packet required to start.
• Universal Precautions required to start.
• References required to start.
• UDS - 10 Panel required to start.
• I-9 Form required to start.
• Signed Job Description required to start.
• Completed Orientation Documents required to start.

Assignment Details

• 40 hours per week.
• Weekend rotation required.
• 8-hour shifts.
