Work Schedule: Full Time, 80.00 hours per pay period (2 weeks)

Pay: $57.50 - $80.50 / hour. Pay is dependent on applicant's relevant experience Summary:

Position Overview

This position is within the Patient Line Refill Center (PLRC) and is non-patient facing. Clinical Pharmacists are responsible for following a standardized CDTM refill protocol approved by the UCHealth P&T Committee to ensure appropriate monitoring and authorizing of medications for UCHealth primary care and specialty clinics. The PLRC team includes Pharmacy Technicians, Medication Access Specialists (MAS), Clinical Nurses and Clinical Pharmacists. They process over 80,000 refill requests per month on behalf of UCHealth providers. Clinical Pharmacists establish collaborative working relationships with clinic staff, providers, and patients across the UCHealth system to authorize, troubleshoot, and resolve prescription refill issues. Additional duties may include identifying patient non-compliance and medication overuse, evaluating medication safety and efficacy based on patient specific data, advocating for utilization of UCHealth pharmacies, and working under collaborative practice agreements to implement medication changes.

Job Duties
• Authorize medication requests using the UCHealth Refill Renewal Protocol.
• Document pertinent patient information related to medication, labs and office visits in the chart.
• Evaluate the impact of patient specific disease states on the safety and efficacy of their medications.
• Recommend appropriate dose adjustments based on applicable laboratory parameters.
• Exercise clinical judgement when reviewing/authorizing medications that do not meet the protocol criteria.
• Work under collaborative practice agreement to switch patients to appropriate medication therapy as necessary.
• Lead a small team of PLRC staff in prioritizing work, meeting performance standards and collaborating with other teams when necessary.
• Serve as a medication expert for PLRC Clinical Nurses, Medication Access Specialist, Pharmacy Technicians, and Pharmacist Interns.
• Participate in updating protocols, workflows and providing clinical education to the team.
• Communicate with providers, patients, pharmacies, and PLRC team members in a clear and professional manner in person, over the phone, and in writing.
• Precept pharmacy students, interns and residents.

Requirements
• Doctor of pharmacy
• Active pharmacist license in the State of Colorado
• At least 1-year experience in ambulatory pharmacy clinical services
• Preferred Qualifications:

o Completion of PGY1 residency training

o Board Certified Ambulatory Care Pharmacist (BCACP) or eligible for board certification within 1-year of hire

Preferred Skills
• Experience working in Epic.
• Proficiency in using all Microsoft Programs (Outlook, Teams, Word, etc.)
• Ability to quickly learn new computer programs.
• Ability to adapt to change
