Minimum Requirements

• Bachelor’s, Master’s, or Doctoral degree in Physical Therapy from an accredited program approved by the APTA or equivalent accrediting body.
• Current, unencumbered South Carolina physical therapy license required to start the assignment.
• Current CPR (BLS) certification required.
• Experience in home health or community-based care preferred.
• 2-3 years of physical therapy experience preferred.
• Familiarity with electronic documentation and basic computer navigation required.
• Must have reliable transportation and a valid driver’s license for home visits.

Assignment Details

• Facility type: Home Health.
• Shift name: Days.
• Schedule: 5x8.
• Role expectations: Perform patient evaluations, provide therapeutic services per physician orders, supervise and evaluate PTAs and home health aides as assigned.
• Must be able to perform home visits and evaluate the home environment as part of the plan of care.
