Full job description
CenterWell Home Health is seeking a Home Health Physical Therapist to deliver compassionate, in-home rehabilitation for seniors and adults with chronic conditions. In this PRN role, you will perform comprehensive evaluations, develop individualized treatment plans, and provide evidence-based interventions to improve mobility, safety, and independence. You will collaborate closely with an interdisciplinary team, educate patients and caregivers, and document care in our EMR.
