Evening Shift LPN
Job Summary:
We are seeking a dedicated and compassionate Evening Shift LPN to join our team at Norwalk Care Center, a leading provider of healthcare services in Norwalk, Connecticut. As an Evening Shift LPN, you will play a vital role in delivering high-quality patient care and support to our residents. As an Evening Shift LPN, you will be responsible for ensuring the health and well-being of our residents within a dynamic and fast-paced environment.
Responsibilities:
Provide direct patient care, including administering medications, wound care, and other treatments as needed
Develop and implement individualized care plans for residents, in collaboration with the healthcare team
Conduct regular assessments and monitoring of residents' condition, reporting any changes or concerns to the Charge Nurse or Physician
Maintain accurate and up-to-date records of patient care and treatment
Participate in inter-disciplinary team meetings and contribute to the development of care plans
Provide emotional and social support to residents and their families
Collaborate with other healthcare professionals to ensure seamless care delivery
