Physical Therapist - Travel Contract

Minimum Requirements
Bachelor‘s, Master‘s or Doctoral Degree in Physical Therapy from accredited schools approved by the APTA
Active State License is Required to Start the Assignment
BLS Certification May Be Required from AHA or ARC
Current Physical Therapy licensure in the state of Kentucky
Current CPR certification
Current driver‘s license and vehicle insurance, and access to a dependable vehicle

Assignment Details
Monday through Friday schedule
Shift hours:
40 hours per week
30 productivity points per week
No on-call requirement
Weekend rotation required
Potential availability on observed holidays is required
Coverage area includes Hardin County, Meade County, and Breckinridge County
May float to other nearby home health agencies as needed
Mileage is reimbursed from the first patient‘s home to the last patient‘s home
