Case Management Registered Nurse

As a Case Management Registered Nurse, you coordinate inpatient and observation care from admission through discharge. You assess psychosocial and medical needs, develop individualized care and transition plans, and use tools such as InterQual to monitor progression of care. Working with physicians, Social Services, and community providers, you help ensure safe transitions, appropriate post-acute services, and clear communication for patients and families.
