CenterWell Home Health seeks a compassionate Home Health Speech Language Pathologist to provide in-home evaluation and treatment for adults with communication, cognitive, and swallowing disorders. You will develop and implement individualized care plans, deliver evidence-based therapy, and educate patients and caregivers to maximize safety, autonomy, and quality of life at home.
