Nurse Practitioner / Physician Assistant – Family Medicine

Responsibilities
- Provide comprehensive primary care to patients across the lifespan
- Conduct physicals, wellness visits, acute visits and follow-up care
- Evaluate, diagnose and manage acute and chronic conditions
- Provide preventive health counseling and patient education
- Coordinate care with specialists and other healthcare team members
- Utilize telemedicine when appropriate
- Maintain accurate and timely electronic medical records
- Current New York State Nurse Practitioner or Physician Assistant license
- Graduate of an accredited NP or PA program
- Family medicine or primary care experience preferred
- Strong clinical, communication and interpersonal skills
- Commitment to patient-centered, compassionate care
- New graduates with a strong interest in family medicine are encouraged to apply
