In-Home Nurse Practitioner / Physician Assistant

Signify Health seeks an In-Home Nurse Practitioner or Physician Assistant to perform comprehensive health assessments for mostly Medicare members in their homes. You will complete detailed histories and physicals, reconcile medications, identify care gaps, and document findings in the EMR to support value-based care. Collaborating virtually with Signify’s interdisciplinary teams, you’ll use data and clinical guidelines to guide decisions, educate patients and caregivers, and improve quality outcomes for vulnerable populations in a flexible, field-based role.
