Signify Health is seeking a Physician Assistant for in-home visits to perform comprehensive health evaluations that support value-based care. In this role, you will travel to patients’ homes in the Dallas area to conduct health risk assessments, review medical histories, perform focused exams, and identify gaps in care. You’ll document findings in our technology platform, develop care recommendations, and coordinate with health plans and providers.
