Full job description
FOX Rehabilitation is seeking a dedicated Physical Therapist to join our clinician-owned and operated private practice specializing in geriatric rehabilitation. As a Physical Therapist with FOX Rehabilitation, you will provide evidence-based, patient-centered care to older adults across a variety of on-site settings, focusing on maximizing functional independence, safety, and overall quality of life. You will perform comprehensive evaluations, develop and implement individualized plans of care, and deliver therapeutic interventions including therapeutic exercise, balance and gait training, functional mobility training, and manual therapy as appropriate. In this role, you will collaborate closely with an interdisciplinary team, communicate regularly with referring providers and caregivers, and educate patients and families on home exercise programs, fall prevention, and strategies to support long-term functional outcomes. You will utilize objective outcome measures and thorough documentation to track progress and support clinical decision making, while adhering to all regulatory and professional standards.
