CenterWell Home Health is seeking a PRN Occupational Therapist to provide compassionate, in-home therapy to seniors and adults with chronic conditions. In this role, you will assess functional abilities, develop individualized care plans, and deliver interventions that improve safety, independence, and quality of life at home. You’ll collaborate with an interdisciplinary team, educate patients and caregivers on adaptive techniques and equipment, and document care using electronic health records.
