In-Home Physician Assistant

Signify Health is seeking a Physician Assistant to deliver high-quality, in-home clinical evaluations that support value-based care. You will perform comprehensive health assessments, review medical histories, conduct focused exams, order and interpret basic diagnostics, and document findings in our technology platform. Partner with physicians, nurses, and care coordinators to close care gaps, optimize chronic disease management, and connect vulnerable patients with needed services. You’ll practice autonomously in the field while supported by a collaborative, data-driven virtual team committed to improving outcomes and reducing costs.
