Dialysis Medical Social Worker

Satellite Healthcare, Inc., a mission-driven dialysis provider, seeks a Dialysis Medical Social Worker to support patients with chronic kidney disease. In this role, you will deliver psychosocial assessments, counseling, and care planning for in-center and home dialysis patients, coordinating with nephrologists, nurses, and dietitians. You will connect patients and families to community resources, support adherence to treatment, facilitate education and support groups, and advocate for patient needs.
