CenterWell Home Health seeks a Home Health Registered Nurse for a 7on/7off schedule providing skilled in-home care to seniors and adults with chronic conditions. The RN completes comprehensive assessments, develops and updates care plans, administers medications and treatments, and delivers wound care, IV therapy, and disease management education. You’ll coordinate with an interdisciplinary team, document care accurately in EHR/OASIS, and communicate closely with patients, families, and physicians.
