Key Responsibilities

• Implement treatment plans developed by licensed Physical Therapists with precision, focusing on improving mobility and supporting orthopedic recovery in residents within ALFs.
• Provide one-on-one care with empathy and attention to detail, while ensuring a patient-first approach in every interaction.
• Monitor patient progress and document responses to treatment, ensuring detailed reporting aligns with care objectives.
• Work as part of an interdisciplinary team focused on coordinated care and optimal patient outcomes, aligning therapy goals with individual needs.
• Build trusting relationships with residents and foster collaboration with caregivers and facility staff to ensure a holistic approach to care.
• Maintain accurate and timely documentation using our user-friendly mobile EMR system, ensuring compliance and ease of information sharing.

Patient Population / Client Focus

In this role, you will work directly with older adults residing in assisted living facilities, providing essential physiotherapy services tailored to their unique health needs. Qualifications

• An active Physical Therapist Assistant license in New Jersey is required.

Compensation and Benefits

At St. Elara Medical Center, we offer a competitive salary range of $60,000 to $75,000 per year, commensurate with your experience and qualifications. In addition to a competitive hourly wage, employees are also eligible for bonus potential based on performance metrics.
