Full job description
Signify Health seeks a Physician Assistant to perform comprehensive in-home health assessments for vulnerable patient populations. You’ll obtain histories, perform physical exams, review medications, and document findings to support accurate risk adjustment and care planning. Partnering with interdisciplinary teams, you’ll identify gaps in care, educate patients, and coordinate follow-up with primary and specialty providers. Using digital tools and analytics, you’ll contribute to value-based care initiatives while working within a collaborative, mission-driven, and flexible environment focused on improving outcomes and reducing costs.
