PHYSICIAN

HarmonyCares seeks a Physician to provide home-based primary and palliative care for medically complex, home-limited adults. In this physician-led, interdisciplinary model, you will conduct in-home and telehealth visits, manage chronic conditions, coordinate with nurses, social workers, and caregivers, and reduce avoidable hospitalizations. You will develop person-centered care plans, ensure accurate documentation, and collaborate closely with community partners and specialists.
