Signify Health is seeking an In-Home Nurse Practitioner or Physician Assistant to deliver value-based care within our Healthcare & Medical Services team. In this field-based role, you will perform comprehensive in-home assessments, review medical histories and medications, and create data-informed care plans for vulnerable and chronically ill patients. You will document visits in our technology platforms, collaborate with interdisciplinary teams, and educate patients and caregivers.
