Evaluate functional mobility, develop and implement individualized care plans, and provide therapeutic exercises, gait and balance training, and home safety recommendations. Collaborate closely with nurses, physicians, and caregivers to optimize outcomes and promote independence. Document visits accurately and timely in EMR while adhering to safety and quality standards.
