Job Description:

Home Health Licensed Practical Nurse (LPN)

PRN | Beverly to Peabody
$32–$38 per hour, based on experience

What You’ll Do

  • Provide skilled nursing care under the direction of an RN and physician

  • Follow individualized plans of care and physician orders

  • Monitor patients and report changes to the RN Case Manager

  • Educate patients and caregivers on medications, treatments, and self-care

  • Complete accurate and timely clinical documentation

  • Collaborate with the care team to improve outcomes and help prevent avoidable hospital visits

What You’ll Need

  • Current Massachusetts LPN license

  • Graduate of an accredited nursing program

  • At least one year of home care experience

  • Valid driver’s license, reliable transportation, and current auto insurance

  • Ability to travel throughout Malden, Lowell, and surrounding communities

  • Home health or hospice experience preferred

  • Ability to sit, stand, bend, lift and move intermittently and lift 50–100 lbs.
