Job Description:

Breast Nurse Navigator- (RN)

Location: Bohemia, NY

Hours: 5/8.5 hour days, Mon-Fri 7:30a-6:30p
Organization: New York Breast Health (NYBH)

In this role, you will:

  • Provide education regarding biopsy preparation, post-procedure expectations, and follow-up
    instructions via phone call
    ● Review, track, and document pathology results, ensuring all findings are communicated to the
    appropriate radiologist for review.
    ● Notify patients of all benign pathology results and ensure understanding of recommendations.
    ● Assist patients in navigating follow-appointments and referrals and ensure all necessary orders are
    placed correctly in the EMR.
    ● Conducting outreach to patients requiring additional diagnostic imaging or follow-up after
    mammography screenings.
    ● Communicate with and assist patients who are due or overdue for annual mammography screenings.
    ● Maintain accurate and timely documentation in the EMR.

We require:

  • Graduate from an accredited School of Nursing and hold an unencumbered New York license.
    ● Nursing Experience which one has acquired clinical competence preferred
    ● Excellent assessment, communication and organizational skills
    ● Good computer knowledge
    ● Bi-Lingual, English/Spanish a plus
    ● HemOnc experience preferred
