$31.00 - $46.25Position Highlights
Home Hospice Nurse for Sioux Falls. On call approximately 2 times a month. Service are is 40 mile radius of Sioux Falls and 30 mile radius of Madison and Flandreau.

A Brief Overview
Responsible for utilizing the nursing process to provide nursing care for Avera@Home patients following the ANA Standards of Clinical Nursing Practice. The Nurse assumes responsibility for the outcome of nursing care provided to patients in their homes or in facilities. The Nurse demonstrates the knowledge and skills necessary to provide exceptional care for the population served.

- Collects patient health data.
- Analyzes the assessment data in determining nursing diagnoses/problem focus.
- Identifies expected outcomes/goals individualized to the patient.
- Develops a plan of care that prescribes interventions to attain expected outcomes.
- Implements the interventions identified in the plan of care.
- Evaluates the patient’s progress toward attainment of the outcomes
- Systemically evaluates the quality and effectiveness of nursing practice.
- Collaborates with the patient, significant others, and healthcare providers in providing patient care.
- Utilizes research/evidence-based findings in practice if appropriate.
- Considers factors related to safety, effectiveness, cost in planning, and delivering patient care.

Required Education, License/Certification, or Work Experience:
- Drivers License - Licensing Board This position requires a valid driver’s license and that the employee is insurable by the Avera Health automobile liability insurance carrier Upon Hire
- Registered Nurse (RN) - Board of Nursing An active license in the state of practice. Upon Hire
Preferred Education, License/Certification, or Work Experience:
- Minimum two (2) years related experience and/or training or equivalent combination of education and experience.
