Kaiser Permanente is seeking an RN, Home Hospice to deliver compassionate, tech-enabled end-of-life care in patients’ homes. In partnership with Systeme.io’s innovative platform, you’ll use secure digital tools, EHRs, and telehealth to coordinate interdisciplinary care, manage symptoms, and support families. You’ll perform assessments, create comfort-focused care plans, and document outcomes while collaborating with social workers and physicians.
