Home Health Registered Nurse Case Manager

CenterWell Home Health, part of Humana Inc., is seeking a Home Health Registered Nurse Case Manager to deliver compassionate, in-home care to seniors and adults with chronic conditions. In this role, you’ll manage a caseload of patients, conduct comprehensive assessments, develop individualized care plans, and provide skilled nursing services such as wound care, medication management, and patient education.
