RN Acute Care Transition Specialties

UCHealth is seeking a Registered Nurse for Acute Care Transition Specialties to support safe, patient-centered transitions from hospital to home or post-acute settings. In this role, you’ll coordinate care plans, perform detailed clinical assessments, reconcile medications, and educate patients and families on follow-up care.
