Nurse Navigator

Job Description

The Nurse Navigator assists in coordinating care for patients with cardiac rhythm disorders and serves as a key resource between patients, cardiology providers, electrophysiologists, clinical staff, Cath Lab staff, and scheduling teams. As a single point of contact, the Nurse Navigator will provide medical information, education, and share knowledge regarding internal and external support services and resources.

Key Responsibilities include but are not limited to:  

  • Coordinate care including consultations, follow-up visits, and procedures and provide timely follow-up from procedures.

  • Assist with monitoring message pools and communicate with patients regarding medication changes, pre-procedure planning, and follow-up post procedures.

  • Coordinate scheduling for procedures with clinic scheduler, anesthesia, imaging, etc. and communicate pre and post procedure instructions.

  • Escalate urgent clinical concerns to the appropriate provider for review.

  • Create and maintain system for tracking patient selection for new procedures.

  • Maintain accurate patient information on external portals for procedures

Location: Cardiology

Hours: Full-time/40 hours per week

Required: ADN; RN; BLS/CPR within 90 days of hire

Pay: Based on experience, starting at $35.43 per hour

Responsibilities

Provides education and information to the patient and family. Initiates and documents patient teaching, including family and significant others, based on assessment of needs., Relays information between provider(s) and client(s), which may include but is not limited to communications with patients, families, pharmacies, nursing homes, office staff, insurers, and facilities. At provider’s instruction, communicates provider advice, instructions, orders and results, as applicable, including accurate documentation of exchanges., Schedules patients for diagnostic, therapeutic, referral, or additional office visits., Serves as a resource for community educational events, such as health fairs, screenings, symposiums, and lectures., Supports the patient during difficult decision-making periods. Assists in coordination of end-of-life care for patient and provides emotional support.
