Job Summary
Works alongside the LVN Office Manager to meet annual HCC Recapture Rates and Quality Metric Targets and goals. Responsible for the oversight of the site’s HCC and STAR Quality Measures. Responsible to work daily with various team members and leads the Quality and HCC Capture/Recapture efforts alongside the LVN Office Manager. They will train new members, engage, and direct team members on the processes around Quality and HCC. They will lead the campaigns for patient screenings, outreach efforts such as vaccination compliance and annual visit compliance. Communicates important information to patients, physicians, providers and hospitals. Also, coordinates scheduling between primary care office and specialty to ensure seamless patient care and improving the closure of gaps in care. Lastly, provides support to any area within the clinic as needed such as referrals/authorizations as well as front office duties.
Job Requirements
Minimum Education Requirement:
High school diploma or equivalent preferred. Knowledge of medical terminology. Bilingual skills desired.
Minimum Experience Requirement:
Three (3) years general office/clerical experience preferred. Supervisory experience required. Healthcare experience preferred. Experience working with HCC and Quality Metrics. Excellent customer service and communication skills. Strong charting/documentation, organizational, problem-solving, and analytical skills.
Minimum License Requirement:
Medical Assistant Certification preferred.

Pay Range:
