Hospice RN Case Manager

Optimal Care is seeking a compassionate Hospice RN Case Manager to coordinate and deliver end-of-life care in patients’ homes and facilities. You will manage a caseload of hospice patients, complete comprehensive assessments, develop and update individualized care plans, and provide skilled nursing, pain and symptom management, and patient/family education. Collaborate closely with physicians, social workers, chaplains, and aides, ensuring timely visits, accurate documentation, and regulatory compliance while supporting families through the hospice journey.
