Registered Nurse

Tufts Medical Center is seeking a Registered Nurse for our Acute Care Transition Team in Boston. In this pivotal role, you will coordinate safe, timely transitions of patients from inpatient care to home or community settings. Responsibilities include comprehensive discharge planning, patient and family education, medication reconciliation, and collaboration with physicians, social workers, and community providers. You will monitor post-discharge outcomes, address barriers to care, and support quality and safety initiatives while contributing to our academic, research-oriented, and culturally diverse environment.
