Full job description
CenterWell Home Health seeks a compassionate Occupational Therapist to deliver personalized, in-home therapy that restores function and promotes independence. In this field-based role, you will perform comprehensive evaluations, design and implement evidence-based treatment plans, and educate patients and caregivers on adaptive techniques and equipment. You will collaborate with an interdisciplinary team, document care in our EHR, and help drive quality outcomes.
