LPN (Licensed Practical Nurse) - Fairport, NY

Fairport Rehab & Nursing Center is seeking a compassionate and skilled LPN (Licensed Practical Nurse) to join our team at our state-of-the-art facility in Fairport, New York. About the Role

As an LPN at Fairport Rehab & Nursing Center, you will provide high-quality care to our residents, focusing on their physical, emotional, and social needs. You will work under the supervision of our nursing team to develop and implement individualized care plans, administer medications, and perform various medical treatments. Your expertise will be essential in maintaining a safe and healthy environment for our residents, and you will be an integral part of our interdisciplinary team.

Responsibilities

  • Provide direct patient care, including administering medications, performing treatments, and maintaining accurate records

  • Develop and implement individualized care plans in collaboration with the interdisciplinary team

  • Monitor and report changes in resident condition to the nursing team

  • Assist with admissions, transfers, and discharges

  • Participate in quality improvement initiatives to ensure excellent patient outcomes

  • Maintain accurate and timely records, including medical charts and electronic records

Requirements

  • Current LPN license in the state of New York

  • High school diploma or equivalent required; associate's degree in nursing preferred

  • One year of experience in a long-term care setting preferred

      Compensation and Schedule:

      Pay Range: $30.00 – $34.00 per hour with pay commensurate with certification and years of post-License experience
      Shift Differentials: +$1.50/hr for weekday B & C shifts and +10% more/hour for all weekend shifts
      Available Shifts: Day (7:00am – 3:00pm), Evening (3:00pm – 11:00pm), Night (11:00pm-7:00AM)
      Schedule Options: Full-Time (37.5 hours per week), Part-Time (22.5 hours per week)
      Weekend Availability: Required
      Sign-On Bonus: $3000 Sign-On Bonus for Full-Time LPN's
