What You Will Do
- Practice independently in the community as part of an interdisciplinary care team.
- Conduct comprehensive patient assessments and develop individualized treatment plans.
- Deliver personalized home-based physical therapy services.
- Monitor patient progress and adjust plans of care as appropriate.
- Educate patients and caregivers to support recovery and independence.
- Collaborate with physicians and interdisciplinary team members.
- Manage your own flexible schedule while meeting Guaranteed Flex Pay role expectations.
- Utilize innovative technology to efficiently document patient care.
Qualifications- Minimum of one (1) year of Physical Therapy experience preferred, or successful completion of the VNS Health Rehab Intern program.
- Current New York State Physical Therapist License and Registration required.
- Valid driver's license may be required based on assigned territory.
Pay Range Per Visit:
Bronx & Westchester: Weekday visits - $105 | Weekend visits - $110
