Acute Care RN Navigator

Tufts Medical Center seeks an Acute Care RN Navigator to coordinate patient care across acute care settings. In this role, you will guide patients and families through complex care pathways, conduct clinical assessments, coordinate transitions of care, and collaborate with physicians, specialists, and interdisciplinary teams. You’ll use evidence-based protocols to optimize outcomes, provide patient education, and address barriers to care. Working in a collaborative, academic environment, you’ll support quality improvement initiatives and help advance patient-centered, culturally competent care.
