In this role, you will evaluate functional abilities, develop individualized treatment plans, and deliver evidence-based interventions to help patients regain independence, manage chronic conditions, and improve safety at home. You will collaborate closely with nurses, physicians, and caregivers, document in the EMR, and educate families on adaptive strategies and equipment.
