Physical Therapist - Travel Contract

Minimum Requirements

• Master's or Doctoral Degree in Physical Therapy from an accredited program. (Client requires Masters Degree or Doctorate).
• Active Louisiana Physical Therapist state license required to start the assignment.
• OASIS experience required. Prior home health (HH) experience required.
• BLS Certification from AHA or ARC may be required.
• Willingness to travel within a 50-mile coverage area. (Preferred: willingness to travel to all service areas).
• Preferred: Skills or attributes demonstrating comfort working across dispersed patient homes and varied environments.

Assignment Details

• Facility Type: Home Health.
• Shift Name: Evenings.
• Shift Details: Evenings 2000-0400.
• Shift Schedule Provided by Client: M-F 8:00-16:30 (client-provided schedule; confirm with recruiter if different for this assignment).
• Weekly Hour Guarantee: 40 hours.
• On Call/Weekend Requirement: No.
• Coverage Area: Up to 50 miles in all directions from base location.
• Mileage Reimbursement: Yes, mileage between visits from first patient to last patient will be reimbursed.
