Minimum Requirements

  • Bachelor's, Master's, or Doctoral Degree in Physical Therapy from an accredited program approved by the APTA.
  • Active WV State Physical Therapy License required to start the assignment.
  • BLS Certification from AHA or ARC may be required.
  • Home Health experience preferred. Candidates without home health experience will be considered if they complete the COS-C exam course (OASIS/COS-C) prior to beginning the assignment.
  • Candidate must agree to a 26-week contract.
  • Submit an up-to-date resume to be considered.
  • Preferred: prior experience with outcome measures and OASIS documentation in home health settings.
