OCCUPATIONAL THERAPIST

CenterWell Home Health is seeking an Occupational Therapist to deliver compassionate, in-home care that enhances patient autonomy and quality of life. In this role, you will conduct comprehensive evaluations, design individualized treatment plans, and provide evidence-based interventions focused on improving safety, daily function, and independence. You will educate patients and caregivers, recommend adaptive equipment, and assess home environments. Working within a collaborative, interdisciplinary team, you’ll document care thoroughly, uphold high standards of quality and patient safety, and contribute to a culture of continuous improvement and professional growth.
