CenterWell Home Health is seeking a Speech Language Pathologist to provide in-home evaluation and treatment for adult patients. In this role, you will assess communication and swallowing disorders, develop and implement individualized care plans, and collaborate with an interdisciplinary team to optimize patient outcomes. You’ll educate patients and families, document progress, and ensure continuity of care.
