Kaiser Permanente is seeking an LMSW Case Manager to join our Healthcare & Medical Services team. In this role, you will provide comprehensive psychosocial assessments, develop person-centered care plans, and coordinate services for patients across diverse settings. You will collaborate closely with physicians, nurses, and community partners to ensure continuity of care, provide crisis intervention, and advocate for patient needs. You will use EMR systems and evidence-based social work practices to track outcomes, support discharge planning, and connect patients with community and behavioral health resources.
