In-Home Physician Assistant

Signify Health is seeking a Physician Assistant to deliver in-home clinical assessments and support value-based care programs for vulnerable populations. You will perform comprehensive exams, review medical histories, reconcile medications, and document findings using our technology platform. Partner with physicians, care coordinators, and health plan partners to identify care gaps, educate patients, and support care plans that improve outcomes and reduce costs.
