CenterWell Home Health is seeking a per diem Occupational Therapist to deliver high-quality, in-home therapy for seniors and adults with chronic conditions. In this role, you will perform comprehensive evaluations, develop individualized care plans, and provide evidence-based interventions to improve safety, independence, and daily function at home. You’ll collaborate closely with an interdisciplinary team, educate patients and caregivers, and document care in our electronic systems while supporting a compassionate, patient-first culture.
