Behind every patient who receives exceptional hospice care is an organization that runs with precision, purpose, and heart. At Transitions Hospice, our Director of Hospice Operations is the person who makes that possible.
The Director of Hospice Operations is accountable for day-to-day staffing, personnel, clinical
outcomes, quality, financial performance, and growth across an assigned region, including all
locations and staff within it. The Director exercises hiring, termination, and staffing oversight for the
region, and carries out all operational, clinical, and financial functions under the administrative and
professional supervision of the Administrator, who serves as the Director's supervisor, evaluator,
and regulatory authority for the license under 42 CFR §418.100(b). All Director decisions remain
subject to the Administrator's review and override authority when necessary for regulatory or
license compliance.
What You'll Do:
Growth & Business Development
- Owns the growth agenda for the region — census growth, referral development, and market share expansion — and is accountable for regional growth outcomes.
- Identifies and pursues partnerships and referral relationships (physicians, hospitals, skilled nursing facilities, assisted living, community organizations) across the region.
- Partners with the Administrator and Finance department on budgeting, forecasting, and business planning for the region.
- Owns performance metrics for the region (census, admissions, length of stay, discharge trends, revenue) and reports progress and recommendations to the Administrator.
- Represents the organization in the community and with external stakeholders, strengthening the organization's reputation and presence in the region.
Quality, Compliance & Clinical Oversight
- Leads quality, clinical, and compliance outcomes across the region under the administrative and professional supervision of the Administrator, ensuring goals and objectives for hospice services meet and promote the organization's quality standards and philosophy.
- Ensures the region's full compliance with federal, state, CHAP, and CMS regulations, laws, standards, and protocols, operating under the direct administrative and professional supervision of the Administrator, who holds ultimate regulatory responsibility and retains authority to direct corrective action.
- Leads continuing quality assessment and performance improvement (QAPI) activities across the region as part of a single, unified QAPI program maintained under the Administrator's oversight; monitors occurrences, complaints, negative trends, and patient/family satisfaction scores, and brings findings and proposed corrective action to the Administrator for review and approval.
- Establishes quality benchmarks, tools, and best practices for consistent standards across all sites within the region, subject to the Administrator's review and approval, so the QAPI program remains centralized and traceable to the certified location.
- Ensures interdisciplinary care is provided consistently across the region, and that clinical and non-clinical staff remain current with hospice regulations, clinical practices, and industry trends.
- Escalates compliance concerns identified through QAPI, complaints, audits, or otherwise, through the hospice's compliance program to the Administrator and, where applicable, the compliance officer/governing body, independent of and without regard to regional growth or financial performance goals set by the Administrator, consistent with OIG General
- Compliance Program Guidance on segregating compliance oversight from operations and financial leadership.
Culture, Talent & People Leadership
- Directly leads, develops, and evaluates the Clinical Leaders across the region, building a strong, engaged clinical leadership team.
- Owns culture across the region: models and reinforces the organization's values and patient-care philosophy consistently across all sites.
- Owns employee selection, onboarding, performance evaluation, and staff development for all Clinical Leader and site staff within the region; ensures adequate and appropriate staffing across the region.
- Carries out personnel decisions for the region: hires, terminations, suspensions, talent reviews, and grievances under the Administrator's administrative and professional supervision, partnering with Human Resources as needed.
- Builds leadership bench strength across the region through mentoring and development of site-level leaders.
Operations, Finance & Administration
- Directs day-to-day operations across the region, including staffing, scheduling, supplies, equipment, and technology needs at every site.
- Partners with the Finance department and the Administrator on operational planning and budgeting for the region.
- Owns business operations and cost controls across the region to ensure financial stability and performance.
- Ensures accurate, timely data collection and reporting to the Administrator on the region's operational, financial, and growth performance, supporting the Administrator's reporting to the Governing Body.
- Ensures conflict and complaint management processes are in place and resolved effectively across the region.

What You Bring:
Education & Experience
- Bachelor's degree in business administration, health care administration, or related field required; Master's degree preferred.
- 5+ years of progressive hospice or home health leadership experience required, including direct clinical leadership oversight.
- Demonstrated track record of driving census and organizational growth in a healthcare services setting.
- Intimate knowledge of Medicare Hospice Certification requirements, CHAP standards, and state and federal hospice regulations.
- Proven ability to supervise and direct clinical and administrative personnel across multiple sites.
- Strong understanding of performance improvement, quality assessment, and utilization management.
