Encompass Health seeks a Clinical Pharmacist to provide evidence-based medication management for patients in our inpatient rehabilitation hospitals. In this on-site role, you will review and verify orders, perform medication reconciliation and pharmacokinetic monitoring, and collaborate closely with physicians, nurses, and therapists to optimize outcomes. You will counsel patients and families, support safe transitions to home health and hospice, and contribute to formulary management, antimicrobial stewardship, and quality improvement in a nurturing, patient-focused environment.
