Sanford Health is seeking an RN Diabetes Educator to provide expert, patient-centered education and care coordination for individuals living with diabetes. In this role, you will assess learning needs, develop personalized self-management plans, and deliver one-on-one and group education on glucose monitoring, insulin use, nutrition, and lifestyle change. You’ll collaborate with an interdisciplinary team, support quality improvement and innovation, and help advance outcomes for rural and regional communities in a mission-driven, collaborative environment.
