CenterWell Home Health seeks a compassionate Home Health Physical Therapist to deliver one-on-one rehabilitative care in patients’ homes. In this role, you’ll evaluate functional status, develop and adjust individualized treatment plans, and provide evidence-based therapeutic interventions to improve mobility, safety, and independence. You’ll educate patients and caregivers on exercises, equipment, and fall prevention, while coordinating closely with nurses, physicians, and other clinicians.
